Key Responsibilities

• Assess the psychosocial status of patients and families related to terminal illness.
• Conduct social evaluations and develop intervention plans based on findings.
• Maintain clinical records and provide information and referral services.
• Act as a liaison between patients, families, and community agencies.
• Participate in interdisciplinary meetings and quality improvement activities.

Qualifications

• Bachelor’s degree in social work accredited by the Council on Social Work Education.
• Minimum of one year’s experience in healthcare; hospice experience preferred.
• Strong verbal and written communication skills.
• Current CPR Certification and valid driver’s license.
